- Providing an overview of the requirements for the person-centered service planning process and plan as outlined by the 2014 Home and Community-Based Services (HCBS) Final Rule;
- Highlighting promising practices for conducting the person-centered service planning process and developing the plan to accurately reflect what is most important to and for the person; and
- Outlining resources to support states in improving processes for person-centered service planning facilitation and plan documentation.
