• Gainesville 34th Street Primary Care – 5469 SW 34th Street, Gainesville
• Lake City VA Commerce Clinic – 484 SW Commerce Drive, Suite 140, Lake City
• Perry VA Clinic – 1224 North Peacock Avenue, Perry
• Leo C. Chase Jr. VA Clinic (St. Augustine) – 100 Deerfield Preserve Blvd., St. Augustine  

What Tele‑Dermatology is: Tele‑Dermatology lets trained VA staff take pictures of your skin concern at the clinic. A VA skin doctor looks at the pictures and gives guidance on what to do next. This helps you get answers without waiting for a regular in‑person dermatology visit.

It’s easy to use:

• You can call 386‑754‑6400, option 1, to make an appointment.
• Or you can stop by in person at one of the clinics listed above.
• No referral and no copay are needed.

Who is eligible: Veterans with skin issues involving up to three areas of concern.

Why this helps you: Tele‑Dermatology can help you get clear advice sooner. The doctor reviews your images and sends recommendations back to your clinic team. This means you can know your next steps faster, even if follow‑up care is still needed.
