Cement company fined after worker fatally crushed
A cement company has been fined £1 million after a worker was found crushed to death by a 1.6 tonne concrete weight used to lift pallets of 25kg bags of cement.
HSE's investigation found:
- a section of perimeter fencing had been routinely removed, exposing dangerous moving parts of machinery
- the company failed to ensure the health, safety and welfare of its workers, despite safeguards being available

Company fined after worker injured by overturned truck
A groundworks company has been fined after a worker sustained serious injuries when a dumper truck overturned. The worker had been reversing the one-tonne truck when it entered an unprotected excavation and overturned.
HSE's investigation found:
- the company had instructed workers not to wear seat belts while operating one-tonne dumper trucks
- open excavations on the site were not adequately protected to prevent vehicles from entering them
