EQRS Clinical Data Submission Deadline Approaching

The End-Stage Renal Disease (ESRD) Quality Reporting System (EQRS) Clinical Data submission deadline for the June 2026 reporting month is August 31, 2026, at 11:59 p.m. Pacific Time. This deadline applies to all collection types (Hemodialysis and Peritoneal Dialysis) and to all submission methods. The EQRS submission deadlines for all of calendar year (CY) 2026 data are available here for EQRS Clinical Data, Clinical Depression Screening and Follow-Up Assessments, and In-Center Hemodialysis Consumer Assessment of Healthcare Providers and Systems (ICH CAHPS) Attestation.

Dialysis facilities must submit EQRS data by each applicable deadline. Missing these deadlines puts your facility at risk for an ESRD Quality Incentive Program (QIP) payment reduction.

Reports Dashboard Redesign in EQRS

The Reports Dashboard landing page has been redesigned to group similar reports together for ease of navigation. To find a report, click on the down arrow next to the report type. Alternatively, you may click on EXPAND ALL to view all the reports available to you. All reports have been renamed to put the user role at the end of the report name to make it easier to differentiate reports. (Ex: 'CMS Form 2728 and Form 2746 Dashboard' is now 'Form 2728 and Form 2746 Dashboard - CMS'). Users may also search for a report using keywords.

Release 7.58.0.3 OOR code deployment of the Provider Enrollment, Chain and Ownership System (PECOS) is now Live.

The following PI enhancements are included in this release:

• Update to the 855R form to resolve the Fee Service Contractor dropdown display issue.
• Updates to the 855A and 855B forms to resolve the Foreign Address error in the Organization Control section.
• Update to all forms with more than 50+ practice locations to resolve the 404 error.
• Update to all forms to resolve Manage Reassignment error.
• Update to all forms to resolve the issue preventing removal of an uploaded signature.
• Update to payments to resolve the incorrect fee payment redirect page error.
• Update to the 855A form to resolve the issue where Managing Employee role titles were not saving.
• Update to the MER with 50+ practice locations to resolve the formatting error.
