Beginning September 21, 2026, the North Florida/South Georgia Veterans Health System (NF/SGVHS) is improving how we prepare for your Primary Care appointments. To help your care team provide the best possible care, you may now receive a short health questionnaire 24 hours before your visit. If it hasn’t been completed, you’ll receive a reminder 2 hours before your appointment.

Here’s what to expect:

• You may receive the questionnaire by text message or email.
• Complete it before your appointment—it only takes a few minutes.
• Your answers automatically and securely go into your VA medical record so your Primary Care team can review important health information ahead of time.
